I hereby authorize the repair of my vehicle. I agree that DFW Storm Solutions (DSS) is not responsible for loss or damage to this vehicle and/or articles left in the vehicle due to fire, theft, or any other causes beyond its control. I also hereby grant permission to this company’s employees to operate the vehicle for the purpose of testing and/or inspection or transporting. Further, I acknowledge that if closer analysis reveals additional repairs are necessary, my insurance company will be contacted for authorization of any additional repair charges. I authorize DSS to use paintless dent repair on any panels or parts deemed as replacement or conventional repair when possible. Old parts will be thrown away unless otherwise instructed. I authorize DSS to to manufacture access to dents that may not be accessible due to their location on the vehicle. I acknowledge that parts fail from time to time and repairs done by DSS are purely cosmetic and non-invasive, therefore, have nothing to do with the mechanics of the vehicle. The total amount of the repair charges must be paid before the attached vehicle can be released for delivery. If insurance coverage pays either a portion of or the total amount due, I acknowledge that the insurance check/draft must be obtained by me or sent in advance by the insurance company. I also acknowledge that I must make arrangements with any lien holder or other payees to endorse the insurance check /draft prior to the release of the above repaired vehicle. I authorize any and all payments payable directly to DSS for consideration of the repairs made to the vehicle. I agree to be contacted by DSS in the future as it relates to hail storm damage. I hereby authorize DSS to act as power of attorney to sign or endorse checks and/or drafts made payable to me and any release thereto, as settlement for my claim for damage to this vehicle. If I withdraw my vehicle without completed repairs, or my vehicle is deemed a total loss, I agree to pay storage fees, insurance consultation fees, rental costs, administrative fees, and any and all applicable fees. DSS is not responsible for prior damages listed on the vehicle diagram on the back of this page. Additional unrelated prior damage may be found once the vehicle is brought the shop and examined more closely. It will be documented and the insured will be notified. DSS is in no case, responsible for any extra insurance above and beyond liability, for the rental vehicle. An “appearance allowance” or “part allowance” may also be applied to help offset the price of the deductible. To secure payment in amount of repairs, and expressed mechanics lien on the vehicle is acknowledged and I further agree to pay reasonable attorney’s fees and court costs in the event that legal action becomes necessary to enforce this contract. I understand the mechanics lien will not be released until payment has been made and has been received in full and authorize repossession of my vehicle if I fail to make payment. In the event of any oversights on the vehicle, DSS reserves the right to complete repairs and address any further issues. In the event of another weather storm, while my vehicle is in DSS possession, I will be responsible for filing another claim and DSS will waive the deductible. If I have rental insurance, I authorize the use of my rental reimbursement coverage, if built into my policy. Any rental reimbursement will be payable to DSS. I acknowledge I have 24 hours after notification of a car completion to pick up my vehicle or I will be charged a $45 a day storage fee. I agree to pay all toll road costs while the rental vehicle is in my possession within 14 days of received invoice.
